How to file motion of contempt in CA?

I am trying to fill out the papers to file a motion of contempt on my ex-husband. I have been told by a lawyer in a free consultation that him not telling me where he moved to with our children, or not telling me what school they are going to, even though we have joint physical custody and I live in Maine. I told him that I want to move closer to where the kids go to school in California, so that I can see them. He has been ignoring me ever since and not telling me any information. Does anyone know of someone that will be able to help me fill out these forms, without having to pay for the help? I just want to make sure that I do it correctly.
